Bundle the exposed commands into @sentry/mcp-server #1909

Part of #1357. Plan: cli-first-mcp task 7.1 (design, tasks). Depends on #1890.

The package stays. Its slim bundle (#1731) bans command handlers today; under this plan it carries the exposed ones. Keep banning stdio-only and hidden commands, Ink, the terminal renderer, and SQLite WASM, and re-measure the 3.75 MB budget.

Done when

• The bundle test passes: initialize, tools/list, and an execute_sentry_read call against mocks.
