A robot can complete 999 tasks successfully and still require a human supervisor for the thousandth. The supervisor may intervene for only a few minutes, yet the factory cannot safely reassign that person if nobody knows when the next exception will occur. That is the contradiction emerging in physical AI. Machines are becoming better at understanding objects, predicting movement and adapting to unfamiliar environments. Physical intelligence can create commercial value in two distinct ways: by reducing the data and engineering effort required to adapt robots to new tasks, and by improving their operational reliability sufficiently to reduce or reorganise human supervision. Neurorehabilitation is being asked to do something its current operating model was not built to do: deliver more recovery, over longer periods, across more fragmented settings, for patients whose capacity changes from day to day. AI appears to solve the visible problem. It can classify movement, tune stimulation, personalise games, assist robots, summarise records, predict deterioration, and monitor home practice. But the visible problem is not the decisive one. The deeper contradiction is that neurorehabilitation depends on adaptive biological response, while the system still buys and manages it as scheduled service activity. The central question is therefore not whether AI can make therapy more intelligent. Rehabilitation robotics is no longer an exotic proposition. The technology can guide limbs, support gait, resist movement, unload body weight, sense effort, deliver repetitions, record kinematics, provide feedback and extend practice beyond what unaided hands can physically sustain. The scientific premise is no longer fringe: structured, intensive, task-oriented practice can support recovery, and robotic systems can help deliver more of it with greater consistency. None of those achievements settles who must respond when the person does not drink, cannot complete the exercise or needs help the robot cannot provide. That distinction is becoming central to the future of assistive robotics. Physical AI is gaining the capacity to perform useful activities in human environments, but care operates under a different constraint from most automation. Someone remains responsible when a task fails, when circumstances change or when a person needs more than the system can safely deliver. Compression can make reinforcement learning materially cheaper and somewhat faster. Its most valuable clinical use, however, is to increase the amount of evidence produced before a robot is allowed to change its behaviour around a patient.
